About Social Support – Individual

Otway Health’s Social Support – Individual is designed to assist you to participate in community life and feel socially connected to your community.

How it works

Once registered with My Aged Care, a Living at Home Assessment will be carried out by a Regional Assessment Officer where your needs are documented and a Care Plan is developed with you, your family member, or advocate.

Your Care Plan will include details of the CHSP funded Social Support – Individual services which Otway Health will deliver, so that you can participate in community life and social activities.

Activities we offer

A range of activities is offered to ensure variety and stimulation. You will have the opportunity to engage socially in the community and improve and maintain your emotional wellbeing.

Outings are determined by yourself as the client and are to assist you to maintain your independence and self-reliance whilst exploring new and old areas of interest. Individual social activities include:

  • Accompanied shopping/bill-paying
  • Lunches
  • Cooking together
  • Attending appointments
  • Telephone monitoring service

Availability

Weekly, fortnightly or monthly across the year with a short scheduled break over Christmas, New Year.
